Learner's Permit application DL-180

A medical certificate is issued to obtain a driver's license to drive a car or motorcycle (PA driver's license).

You must submit a medical certificate to the DMV (Department of Motor Vehicles). To apply for a certificate, a certified healthcare professional must complete Form DL-180 (Learner’s Permit Application DL-180).

To obtain a certificate (Learner’s Permit Аpplication DL-180) you need to visit our office in Philadelphia, Pennsylvania, where a certified medical professional will fill out all the necessary forms.

A document confirming:

  • First Name and Last Name.
  • Date of Birth.
  • Social security (if not, then not necessarily).
  • Place of residence.

If you have a driver’s license or state ID in Pennsylvania or other states, we need information about your license or ID number.

If you wear glasses, contact lenses and/or hearing aids, be sure to bring them with you.

The certificate is valid for one year from the date of receipt.
